Community Care Services Program (CCSP)

  • Legacy Link is the Lead Agency for this program funded by Medicaid for low income, frail persons of all ages needing nursing home level of care.
  • Nurses perform assessments and social workers arrange for services in homes of persons to help individuals stay in their homes and communities and avoid premature placement into nursing homes.

Senior Community Service Employment Program

  • Persons aged 55+ with income falling within the Federal guidelines may be eligible for the Senior Community Service and Employment Project.
  • Trainees are placed in part-time positions in public and non-profit agencies for training to enable them to enter the workplace.
  • This program is geared to serve low-income individuals.
  • Initial interviews may be done at the Senior Center or through Legacy Link.

Homemaker Services

  • Homemakers provide light housekeeping, grocery shopping, and running errands for the frail elderly.
  • There are no income restrictions for Title III Homemaker Services.
  • Priorities are given to clients assessed with the greatest need following telephone screening and an assessment performed by a Legacy Link R.N.
  • Some homemaker clients are assessed for a cost share based on a sliding scale of income.
  • Funding availability may necessitate a waiting list for Homemaker Services.
